• Requester Name: Enter the name of the person who requested the inspection.
• Source: Enter the source of the request.
• Requester Phone: Enter the phone number of the person who requested the inspection.
• Confirmation Id: This field will automatically populate with a confirmation number.
• Requester Comment: These are comments from the person who requested the inspection.
• Location Details: Enter any details about the location or address, such as landmarks.
